FOR ALL CLIENTS PLEASE NOTE THE FOLLOWING:

-- Make sure you have filled in all the contact and health history information with as much detail as possible in the client portal. There are multiple tabs to complete and lots of details to add, and this MUST be done prior to your visit. If you are having difficulty please call prior to your appointment and we can walk you through it. Patients who have not completed the patient portal and health history will be rescheduled.

--Please sign consents forms under the "Forms and Records" tab.

*If you are just starting prenatal care, please fill out: Prenatal Informed Consent, Financial Agreement, and HIPPA document

--Please add a picture of yourself when filling in the portal information - we love being able to easily put a face to a name when when open up your chart.
